OLYMPIA — The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to [email protected].
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Asotin County
In February 2024 the Board of Nursing (formerly the Nursing Commission) summarily suspended the registered nurse license of John Edward Hulsey (RN60859412) pending further legal action. In December 2023, Hulsey was charged with first-degree voyeurism, a class C felony, in the Superior Court of Asotin. Hulsey allegedly recorded female coworkers in the workplace locker room without consent.
King County
In August 2023 the secretary of health suspended the acupuncturist and eastern medicine practitioner license of Adam Lee Gardner (AC60756013) for at least three years. Prior to petitioning for reinstatement, Gardner must undergo a complete psycho-sexual evaluation. In February 2020, Gardner touched a patient’s genitals, breasts, and pulled the patient’s blanket and underwear down without consent.
Snohomish County
In February 2024 the Pharmacy Quality Assurance Commission summarily suspended the certified pharmacy technician license of Rachel H. Langdon (VA00053093) pending further legal action. Between August 2022 and March 2023, Langdon allegedly diverted oxycodone, a controlled substance.
In February 2024 the secretary of health released Sujin Rhi (AC60920205) from the terms and conditions on her acupuncturist or acupuncturist and eastern medicine practitioner license.
In February 2024 the Chiropractic Quality Assurance Commission indefinitely suspended the chiropractor license of Kyle Matthew Osborne (CH60450085). Prior to petitioning for reinstatement, Osborne must pass an ethics course, pay a $15,000 fine, and provide his plan for patient notification of his business interests and for complying with the anti-rebate statute. Osborne was the sole owner of a chiropractor business where his employee was referring patients to another business in which Osborne was co-owner. The patients were not notified of Osborne’s financial interest in the referrals to the other business.
Spokane County
In February 2024 the secretary of health suspended the registered agency affiliated counselor and substance use disorder professional licenses of Steven C. Floyd (CG60153079, CP60432152). Floyd may not petition for reinstatement for at least three years. Prior to petitioning for reinstatement, Floyd must pass an ethics course and pay a $5,000 fine. Floyd did not respond to a statement of charges regarding his engagement in sexual misconduct with patients including dating a patient and coercing a patient to engage in sexual acts.
